How do I complete a MyMedicare patient Registration?
1) You can complete your MyMedicare registration via Medicare online services:
There is a MyMedicare tab within these online services. Simply select MyMedicare and follow the prompts.
It will ask you for the following information:
- Practice Name: Diamond Creek Medical Centre
- Suburb/Town: Diamond Creek
- Postcode: 3089
- Your preferred GP: (General Practitioner)
Complete the details above and submit your registration. Once received Diamond Creek Medical Clinic can accept your MyMedicare registration.
Please note:
DVA Veteran Card Holders: If you hold both a DVA Veteran Card and a Medicare card, you can register using either. You can only have one registration, which will apply to any relevant Medicare and/or DVA-funded services, regardless of which card you use to register.
If you choose to register in MyMedicare with a DVA Veteran Card, you will need to complete a registration form at Diamond Creek Medical Center.
Children under 14 years: A parent/guardian will need to register a child under 14 years and provide consent on their behalf.
Young people aged 14 to 17 years: Young people aged 14 to 17 years will be able to register and provide consent without a parent/guardian. It is important to note that registration for a child under 14 years will need to be completed at Diamond Creek Medical Centre.
2) Registration in-person at Diamond Creek Medical
I am unable to register via my Medicare online services account, what should I do?
You can complete your MyMedicare registration one of the following ways:
- Download and complete your registration form at home and hand it in to our team
- Fill out a registration form at the practice and sign the form to provide consent.
Our team can start your registration process in MyMedicare. You will then need to compete and provide consent for your registration in your Medicare online services. Diamond Creek Medical Centre will then enter your registration details into the MyMedicare system to complete your registration.

MyMedicare – your questions answered
The goal of MyMedicare is to strengthen the relationship you have with your preferred Doctor and medical centre.
Attending the same clinic and seeing the same GPs for medical appointments will allow your chosen doctor to provide you with better ongoing care. You can still attend other clinics, however, you will receive some extra benefits at your chosen clinic. If you want to choose our medical centre as your ‘home clinic’, you will need to register with MyMedicare, select our medical centre and choose your preferred GP. We welcome our current and past patients to choose our medical centre as their preferred home clinic.
Benefits of registering for MyMedicare with your GP supported by Diamond Creek Medical Center include:
- Greater continuity of care with your registered practice.
- Helping to improve your health outcomes
Registration in MyMedicare will initially provide the most benefit to people who need access to longer consultations with their GP, for example older Australians, people who seek mental health care from their GP and people with chronic and complex health conditions.
Other benefits you will receive include:
- All registered patients will have access to longer MBS funded telehealth consultations (telephone calls) with GPs at your registered ‘home clinic’(Levels C & D). This means you and your GP can have more in-depth conversations (over 20 minutes) about your health. Medicare will provide a larger rebate for these longer telehealth consults to help make it more affordable for you, but only at your registered home clinic.
- From November 1, the Australian Government is introducing a ‘Triple Bulk Billing’ incentive with GPs bulk bill children 15 and under, pensioners and concessions card holder to help subssidise the rising costs of health care. This incentive may also apply to longer MBS telehealth consultations (Levels C, D and E) with your GP at our registered home clinic.
These initial benefits for registered patients will be expanded by the Australian Government over time, so registering our medical centre as your preferred’ home clinic’ is important.
